ENROLLMENT AND WITHDRAWALS

If you are returning to LBUSD please email me the above information and gather the items listed below:
- Transcript
- Birth Certificate
- Immunizations
- 2 proofs of residence (see below)

Residency Verification
(EC 48204.1)A parent/guardian must offer documentation that reasonably verifies that his/her residence is located within district boundaries.
Acceptable documentation of a residence includes two of the following:
o Property tax payment receipts
o Rental property contract, lease or payment receipts
o Utility service contract, statement or payment receipts (Gas, Water or Electric. No Phone or Cable)
o Pay stubs
o Voter registration
o Correspondence from a government agency
If you are new to LBUSD you will need to provide the following items:

Withdrawals:
If you are wishing to withdraw from Millikan:
Our district requires that we receive proof of acceptance from the new school before we can release. This could be correspondence between you and the new school about enrollment, a letter of acceptance, or possibly request for records from the new school. This is a common practice amongst many districts, now including LBUSD.
