ATHLETIC CLEARANCE

In order to participate in Millikan Athletics, a parent/guardian and student must complete the online clearance process.

 
Athletic Clearance During the School Year:   

To begin the sports clearance process for the 22-23 school year please follow the steps below:

1. Create an account on www.AthleticClearance.com (Athletic Clearance Guide)
    
2. Print out the Confirmation Page, please be sure to sign and date
    
3. Provide the completed Physical Examination Form and attach with the signed Confirmation Page
    
4. Submit the Confirmation Page & Physical Exam form to the front office (Athletic Clearance Basket located at the front counter)
    
5. Once the office receives your packet the clearance will start (submitting incomplete paperwork can delay the process)
    
6. Clearance process can take up to 2 weeks – Due to the high volume of clearance requests we are unable to process last minute requests

Medical Insurance Required
In order to participate in athletics, all student-athletes must have a valid medical insurance coverage. If you do not have current medical insurance, you may purchase a plan through Myers-Steven-Toohey Insurance.

Academic Eligibility
Each student-athlete must have a minimum of a 2.0 GPA on the report card received at the close of the last grading period. Please check with your counselor or Athletic Director to determine your academic standing.
