ATHLETIC CLEARANCE

In order to participate in Millikan Athletics, a parent/guardian and student must complete the online clearance process.

 

Summer Sport Session:

  • Students complete the hard copy forms provided during sport orientation day.  If you were not able to attend sport orientation, we have attached the summer sports packet and the blank physical exam form, in case you need a new physical for your child. 
     
  • Please Complete all forms and have your child turn that in with a COPY of their most current physical, which must be valid through the summer session.  Please retain the original copy of your physical provided by your doctor to submit with their paperwork (as described below) for their team(s) during the 2022-23 school year.
     
  • Parents must provide their own copy of a physical with the summer packet forms.  We cannot look for physicals submitted to middle schools.  If you did not retain a copy of your child's physical, please contact your doctor's office and submit the Physical Exam Form attached.  The doctor's office will fill that out for you without an exam needed, so long as the child's physical has not expired.

  • To begin the process of sports clearance for your child during the 2022-2023 school year, please begin by creating an account on athleticclearance.com.  Please remember you MUST print out a confirmation page for EACH sport and attach it to a copy of their sports physical.
     
  • These can be turned in to the basket in the main office.  This is ONLY for sports clearances for 2022-2023, not summer.  Summer packets will be turned in to the coach and returned to you at the end of summer.
     
  • Parents must provide their own copy of a physical with the confirmation page(s).  We cannot look for physicals submitted to middle schools.  If you did not retain a copy of your child's physical, please contact your doctor's office and submit the Physical Exam Form attached.  The doctor's office will fill that out for you without an exam needed, so long as the child's physical has not expired.  

 

Medical Insurance Required
In order to participate in athletics, all student-athletes must have a valid medical insurance coverage. If you do not have current medical insurance, you may purchase a plan through Myers-Steven-Toohey Insurance.

Academic Eligibility
Each student-athlete must have a minimum of a 2.0 GPA on the report card received at the close of the last grading period. Please check with your counselor or Athletic Director to determine your academic standing.
